If you notice that we have made
a mistake regarding your insurance information please
do the following:
Fax a copy of the front and
back of your insurance card to 954-370-1533. On the
fax make sure to neatly spell your name and list your
account number which is located on the center/right
portion of your bill. Your account number will start
with the letters DS
